High
School Transition
When
students with disabilities exit school at 21, they must rely on
a combination of services and supports from different agencies and
community resources. Eligibility and requirements for adult
services differ from those offered through the school district.
Planning for on-going support prior to exiting high school is key
to a successful transition.
