Complaints are received through the front office staff in a variety of
ways; by phone, by e-mail, or in person. However, if in person, confidentiality may be difficult as others may be able to overhear the complaint.
The information you provide must be sufficient in order for us to forward the complaint to the appropriate field staff. The field staff will prioritize their workload and perform a follow-up investigation.
For Complaints regarding Drinking Water, Sewage, or Solid Waste (Garbage), please provide the following information:
- WHO the violator is, if known;
- Specific details of WHAT the complaint is regarding;
- WHEN the violation occurred, if known;
- WHERE the violation occurred - a specific address is very helpful for locating a problem site; and
- YOUR NAME and a daytime CONTACT NUMBER for follow-up or additional questions to complete the investigation.
For Complaints regarding Food-related Illness, please provide the following information.
- NAME of the food establishment;
- DATE and TIME of visit;
- YOUR NAME and a daytime CONTACT NUMBER for follow-up or additional questions to complete the investigation;
- Detailed information regarding WHAT each person ate, regardless of whether they became ill or not; and
- SYMPTOMS and DURATION of illness for each person who became ill.
For Complaints regarding Food-related Sanitation(Cleanliness), please provide the following information:
- NAME of the food establishment;
- DATE and TIME of visit;
- YOUR NAME and a daytime CONTACT NUMBER for follow-up or additional questions to complete the investigation; and
- Detailed information regarding WHAT the sanitation issue is.
